How do I add my Chronic Disease Management Plan details?

How to add your Chronic Disease Management Plan details referred by your GP

If your practitioner has booked an appointment that is partially claimable by MEDICARE, you will be required to enter your Medicare card details as well as the referral details as provided by your GP.

You may have been provided a referral form and the information on this form that is required to process your claim includes:

  • Provider (GP) Name

  • Provider Number

  • Date of referral/signed
    ​

To add this information to your appointment, follow these steps:

  1. Click APPOINTMENTS

  2. Click on the upcoming appointment that has the Medicare logo
    ​

  3. Click ADD MEDICARE CARD

    1. Add your First name as it appears on your card

    2. Add your Last name as it appears on your card

    3. Add your date of birth

    4. Add your Medicare number

    5. Add your Medicare Reference number

    6. Click SAVE
      ​

  4. Click ADD REFERRAL

    1. Add Provider (GP) name

    2. Add provider number

    3. Add referral date

    4. Click SAVE
      ​
