You must use the same email address associated with your HealthBank account or provide the email address associated with your HealthBank account in the body of the email.
We will get back to within 2 business days to confirm we have disabled your account and removed you from any mailing lists.
We will then remove any personal data from our database within 30 days.
Please note, you have a relationship with any practitioners that you have connected with. If you would like your data to be removed from their records, you need to directly email them a separate request to do so.
HealthBank is not able to stop practitioners contacting you as you have a separate relationship with them.
