How to complete an intake form/digital form

This article outline the steps on how to access your digital forms sent by your practitioner

Your practitioner may have sent you an intake form or other digital form to fill out. You can access and complete the form through the web portal or the mobile app. Outlined below are the steps to do this:

Web Portal

  1. Log in to your portal with your email address and password - https://www.healthbank.io/users/sign_in

  2. Click on the "Notification bell" in the top right of your profile. The notification title will be "A practitioner has shared a form with you". Click on this notification.

  3. You will also receive an email notification titled " [Practitioner Name] has shared an intake form with you", which will include the link to the form. You can click on this link and it will take you to your form.


  4. Answer the relevant fields and questions and click Next.


  5. When you complete the last section, click Save in the bottom right corner.


  6. Once you save the form, your practitioner will be notified that you have completed it.

Mobile App

  1. Download the mobile app from your choice of store:

  2. Log in to the mobile app.

  3. There will be a notification on your Dashboard (home screen) stating "Your practitioner has sent you an intake form". Click Get Started to fill out the form.


  4. After you have completed a question, click Next.


  5. Once you reach the end of the form, click Submit.

  6. Once you submit the form, your practitioner will be notified that you have completed it.
