How do I create a custom question for my forms?

If a question you need isn't in the question library, you can create your own.

1. Click Forms in your navigation menu on the left.
2. Click the "+" symbol if you are creating a new form.
   
3. Enter a Title for the form.
4. Enter a Description for the form.
5. Click Continue.
   
6. Select your questions from the question library.
7. To create a Custom question, click into one of the categories of health.
8. Click + Custom question.
   
9. Enter your Question.
   
10. Select the Type of response from the dropdown.
    
11. Type your Answers. Press Enter if you have multiple answers to choose from.
    
12. Click Save.

Your custom questions are saved in the Health category you created it in.
