How do I create custom questions?

If the question you need is not in the form builder question bank you can create a custom question

How do I create a new custom question?

Create a new form, or go to an existing form

Scroll to the bottom of the question bank

Add a custom question

Add your question and select the response type:

Select which category the question will be added to in your forms (this will impact the order in which your questions will be presented to your patient.

If your question response type is drop down or multiple choice, add the possible answers

Save your question, this will automatically add it to the form you are creating

You can find your previously created custom questions in the custom questions category
