How do I create and edit a digital form?

1. Login to you practitioner account
2. Click FORMS tab in your menu bar
3. Click the "+" symbol to add a new form
   
   
4. Create a TITLE and a DESCRIPTION for the form
5. Click CONTINUE
   
   
   
6. From the question library, click on the health category to access the list of questions to choose from
   
   
7. Check the questions you wish to add to your FORM.
   
   
8. If you wish to add your own custom question, click + CUSTOM QUESTION
   
   
9. Type your question
10. Select if you wish the response to be: Free text, a date selector, single select or multi-select  
    
11. Type your answers and press ENTER to create new answers
    
12. Click SAVE
13. Once you have chosen your preferred questions and created any custom questions, click SAVE
14. Your form will be saved as a template in your FORMS section which you can edit at any time

How to EDIT a form

1. Login to you practitioner account
2. Click FORMS tab in your menu bar
3. Click on the form you wish to edit
4. Click EDIT
   
5. Click on the category of health which you have asked the questions from
6. UNCHECK the green ticks for any questions you wish to remove
   
7. Click SAVE
   
   
8. Click SAVE FORM
