If your patient does not answer the form before your appointment, you can fill in the answers with them in the appointment.
How to answer the form with my patient
Navigate to the patient's file using the left hand navigation menu Patients > select patient and scroll to forms
Select the form and start adding the answers with your patient:
One you have filled in the form, save it
NB once you confirm that you want to complete the form, you will not be able to edit further:
How does my patient see their answers?
The responses will be saved to the form and also to the patient's HealthBank Record
NB the patient can update their answers anytime on their HealthBank record. This will not update your form copy in the HealthBank mobile app
